İyatrojenik hipoparatroidiye sekonder baş dönmesi ve ellerde kasılma yakınması ile acil servisimize başvuran Fahr sendromu olgusu

Abstract

Fahr sendromunun etyolojisi kesin olarak bilinmemektir. Bu sendroma hipoparatiroidizm, psödohipoparatiroidizm veya hiperparatiroidizm neden olabilir. Fahr sendromu serebellum, talamus ve bazal gangliyonlarda kalsiyum ve fosfor metabolizma bozukluğu sonucu oluşan kalsinozis ile seyreden nörodejeneratif bozukluklarla karakterize bir hastalıktır. Beyin bilgisayarlı tomografisinde kalsiyum ve çeşitli minerallerin birikimi gösterilebilir. Bu yazıda, baş dönmesi ve ellerde kasılma nedeni ile acil servisimize başvuran ve yapılan muayenesinde hipokalsemi saptanan ve öyküsünden 15 yıl önce subtotal tiroidektomi ameliyatı geçirmiş olduğu öğrenilen 68 yaşındaki erkek hasta sunuldu. Olguda hipoparatroidizme bağlı Fahr sendromu düşünüldü.

The etiology of Fahr syndrome is precisely not known. Hypoparathyroidism, pseudohypoparathyroidism or hyperparathyroidism may give rise to this syndrome. Fahr syndrome is a disease characterized by neurodegenerative disorders that present with calsinosis in the cerebellum, thalamus and basal ganglia as a result of calcium and phosphorus metabolism defect. Deposition of calcium and other minerals can be indicated on brain computed tomography. In this article, we present a 68-year-old male patient, who had undergone subtotal thyroidectomy surgery 15 years ago, admitted to the emergency department with dizziness and contraction in the hands, and we detected hypocalcemia on his examination. We considered Fahr syndrome due to hypoparathyroidism.
