Akne skarların tedavisinde KTP, ND-YAG lazerler ile birlikte derma elektroporasyon kullanımı ve sonuçların değerlendirilmesi

Abstract

Akne skarlarında KTP / Nd-YAG lazerler ile derma elektroporasyon yönteminin birlikte kullanımının etkinliğini araştırdığımız çalışma 2007 mayıs ayı ile 2010 kasım ayı arasında kliniğimize gelmiş 63’ü kadın, 6’sı erkek toplam 69 hasta üzerinde gerçekleştirildi. Yaşları 15 ile 43 arasında değişmekte olan olguların ortalama yaşı 30,0, erkek hastaların yaş ortalaması 20,2 , kadın hastaların yaş ortalaması 31,0 idi. Akne skarların derinliğini azaltmak, deri yapısını, gözenek büyüklüğünü düzeltmek ve mikro vaskularizasyonu tedavi etmek için Nd-YAG lazeri ile, küçük damarlar, pigmenter lezyonları tedavi etmek ve deriyi daha canlı hale getirmek için de KTP lazeri kombinasyonu uygulanmış hasta sayısı 30 idi. Lazer uyguladığımız hastalardan biri tedaviyi tamamlamadı. Sadece derma elektroporasyon uygulanmış 2 kadın hastamız vardı. Daha iyi bir sonuç almak amacıyla hücresel aktivitenin, fibroblastların sentez kapasitelerinin, yeni kollajen, elastin ve hyaluronik asit üretiminin arttırılması, derinin neminin, parlaklığının ve sıkılığının sağlanmasına daha fazla etkili olması için bazı ilaçların transdermal iletimini sağlayan derma elektroporasyon yöntemi ile iki lazer kombinasyonu birlikte uygulanmış hasta sayısı ise 37 idi. Kombine tedaviyi tamamlamayan 6 hasta vardı. Sonuç değerlendirmelerine 3-4 ve 5-6 tedavi sayısını tamamlayan hastalar alındı. Derma elektroporasyon tedavisi alan hasta sayısı 2 kişi olduğu için sonuç mükayese değerlendirmesine alınmadı. Bu uygulamalardan aldığımız cevapların değerlendirmelerinin ortalama yüzde sonuçlarına baktığımızda; hiç cevap vermeyen hastamız olmadığı gibi tama yakın iyileşme yani tamamen skarın kaybolduğu bir cevap gözlemlenmedi. Her iki tedavi grubunda tedavi sayısı arttıkça iyileşme oranlarında da yükselme gözlemlendi. Hastalarımızın büyük çoğunluğu sonuçtan memnun olduklarını ve ciltlerinin gerginliğinin, canlı görünümlerinin devamını sağlamak için zaman zaman tekrar aynı tedaviyi görmek istediklerini bildirdiler.

The work, where we have researched the effectiveness of use of KTP / Nd-YAG lasers and derma electroporation method together on acne scars, has been performed on total of 69 patients that have visited our clinic between May of 2007 and November 2010, of which 63 were women and 6 men. The average age of the cases, whose ages varied between 15 and 43, was 30.0, average age for male patients was 20.2 and 31.0 for female patients. Number of patients were 30, on whom the application was made with Nd-YAG laser in order to reduce the depth of acne scars, to restore the skin structure and the porosity size and to treat the micro vascularisation and on whom the application was made with combination of the KTP laser in order to treat the veins and pigmentary lesions and to enliven the skin more. One of the patients, who we have applied laser, could not complete the treatment. We had only 2 female patients, who we have applied derma electroporation on. The number of patients, on whom derma electroporation method, where this provides transdermal transfer of some of the medicaments, was applied together with two laser combination in order to increase the cellular activity, the synthesis capacities of the fibroblasts, to increase the production of the new collagen, elastin and hyaluronic acid and to have more effect for the provision of the moisture, gleam and firmness on the skin, were 37. There were 6 patients, who have completed the combined treatment. Patients, who have completed 3-4 and 5-6 treatment runs, were considered for the evaluations of the results. Since the number of patients were 2, who had derma electroporation treatment, the result had been considered for the comparison evaluation. When we look on the average percentage results of the evaluations that we have had from these applications, it has not been observed that we have had any patient, who has not responded nor a result, where a near total improvement has been achieved, in other words, where the scar disappeared completely. It has also been observed that an improvement has been achieved as the number of treatments were increased for both of the treatment groups. Majority of our patients have stated that they were happy about the results and wished to have the same treatment again from time to time in order to have continuation of the rigidity of the skin and its lively appearance.
