Diyabetik ve Hipertansif Hastalarda Retina Sinir Lifi Düzeylerinin Optik Koherens Tomografi ile Ölçümü

Abstract

Amaç : Diabetik ve hipertansif hastalarda retina sinir lifi tabakası (RSLT) kalınlığını spektral-alan optik koherens tomografi (SD-OKT) kullanarak değerlendirmektir. Metod : Çalışmamıza, 15 diyabetik retinopatisi olmayan diyabet hastasının 30 gözü ve 14 hipertansif retinopatisi olmayan hipertansiyon hastasının 28 gözü dahil edildi. Kontrol grubu olarak refraktif şikayetlerle kliniğimize başvuran 11 hastanın 22 gözü değerlendirildi. Tüm hastaların ve kontrol grubunun her iki gözüne RSLT kalınlığı için OKT yapıldı. Bulgular : Diyabetik olgularda her iki gözde ölçülen en düşük RSLT kalınlığı ortalaması 89,09 ±11 ?m , sağlıklı kontrollerde 104,52 ±11 ?m idi. Hipertansif grupta her iki gözde ölçülen en düşük RSLT kalınlığı ortalaması 90 ±13 ?m olduğu saptandı. Diyabetik grup, kontrol grubuyla karşılaştırıldığında, optik disk çevresinde superior ve inferior alanda RSLT ‘de incelme (p-değeri = 0,002) saptandı. Hipertansiyon ve RSLT incelme korelasyonu diyabetik gruba göre daha düşük (p-değeri=0,04) tespit edildi. Sonuçlar : Bu çalışmada, diyabetik olgularda kontrol grubuna göre RSLT’nin azalmış olması nörodejenerasyonun diyabetik retinopatinin erken evresindeki önemli bir komponent olduğunu düşündürmektedir. Hipertansiyonun RSLT üzerinde anlamlı bir değişiklik yaptığı saptanmamıştır.

Purpose : To evaluate the thickness of retinal nerve fi ber layer (RNFL) in diabetic and hypertensive patients using spectral-fi eld optical coherence tomography (SD-OCT). Methods : In this study, 30 eyes of 15 patients without diabetic retinopathy and 28 eyes of hypertension without hypertensive retinopathy were included. Twenty-two eyes of 11 patients who were referred to our clinic with refractive complaints as control group were evaluated. OCT was performed for the RNFL thickness in both eyes of all patients and the control group. Results : The mean RNFL thickness measured in both eyes was 89.09 ± 11 ?m in diabetic patients and 104.52 ± 11 ?m in healthy controls. The mean RNFL thickness measured in both eyes was 90 ± 13 ?m in hypertensive group. When the diabetic group was compared with the control group, the thinning of the RNFL (p-value = 0.002) was observed in the superior and inferior area around the optic disc. Hypertension and RNFL thinning correlation was lower than the diabetic group (p-value = 0.04). Conclusions : In this study, decreased RNFL in diabetic patients compared to the control group suggests that neurodegeneration is an important component in the early stage of diabetic retinopathy. There was no signifi cant change in hypertension on RNFL.
