Toksik dozda alınan yeni nesil oral antikoagülanların neden olduğu gastrointestinal kanamada tedavi seçenekleri: Tek merkez deneyimi ve olgu serisi

Abstract

Yeni nesil oral antikoagülan ilaçlardan biri olan dabigatran, direkt etkili trombin inhibitörüdür ve renal klirens ile atılır. Dabigatran toksisitesinde uygulanacak tedavi yöntemleri belirsizdir. Bu yazıda, non-valvüler atriyal fibrilasyon nedeniyle dabigatran kullanan ve gastrointestinal kanama gelişen üç hastaya uygulanan faktör konsantresi infüzyonu, taze donmuş plazma infüzyonu ve hemodiyalizi içeren tedavi yöntemlerinin etkinlikleri değerlendirildi. Üç hastadan biri kaybedildi, diğer ikisi iyileşti.

Dabigatran, which is one of the new generation oral anticoagulants, is a direct thrombin inhibitor and is eliminated by renal clearance. The treatment modalities to be used in dabigatran toxicity is unclear. In this article, we evaluated the efficacies of treatment modalities consisting of factor concentrate infusion, fresh frozen plasma infusion, and hemodialysis performed in three patients who were using dabigatran due to non-valvular atrial fibrillation and who developed gastrointestinal hemorrhage. One of three patients died, the other two were healed.
